Our dedicated team is hiring and ready to provide compassionate, high-quality care for patients and families across the region.We are seeking a compassionate and dedicated PRN Hospice CNA to join our interdisciplinary care team. In this role, you'll provide personal care, comfort, and supportive services to patients and families. This position offers flexible scheduling and the opportunity to make a meaningful impact while working alongside a deeply committed hospice team.Key ResponsibilitiesProvide hands-on personal care to hospice patients, including bathing, grooming, dressing, repositioning, and ADL supportAssist with maintaining a safe, clean, and comfortable environment in the home or facility settingObserve and report changes in the patient's condition to the RN Case ManagerOffer emotional support to patients and families with sensitivity and respectDocument care accurately and in accordance with regulatory and organizational standardsParticipate in team communication to ensure coordinated, high-quality careTravel to patient homes, facilities, and communities within assigned coverage areaQualificationsCurrent New Mexico CNA certificationPrefer at least six months direct patient care experience; hospice or home health experience preferredReliable transportation and valid driver's licenseAbility to work independently in home settings with professionalism and compassionStrong communication, time-management, and interpersonal skillsComfort supporting patients and families during end-of-life transitionsWhat We OfferPRN BenefitsFlexible PRN schedulingCompetitive hourly pay ratesSupportive, mission-driven team cultureTraining, mentorship, and continued education in hospice best practicesMileage reimbursement for field visits
