Licensed Practical Nurse

Looking for 2 LPN's for Provides appropriate and accurate dispensing, medical and medical support

services, including information, referral and follow-up to members enrolled.

2 shifts are available: 4 -10 hour and M - F 0445 to 1:45 pm

Essential Duties and Responsibilities

1. As part of an interdisciplinary team, participates in evaluation and

assessment of members.

2. Collaborates with physicians, prescribers, and clinical staff to promote

member recovery.

3. Completes appropriate medical procedures and makes necessary referrals.

4. Coordinates medical care with primary physician and other health care

providers.

5. Observes members behavior patterns and medical symptoms related to

treatment. Reports assessment to provider or clinical staff for follow-up.

6. Provides methadone medication delivery as ordered by physician.

7. Completes daily usage report for methadone at the end of each shift including

Reconciliation of inventory of methadone and Suboxone at conclusion of dosing,

printing, signing and verifying as accurate the daily report.

8. Assures proper operation of assigned equipment including weekly calibration

of pumps and other equipment, assures orders properly entered in dosing software

and adheres to all flags to dosing as entered by providers.

9. Conducts Suboxone inductions as ordered by the BHMP per facility protocol in

accordance with all Federal, State policies.

10. Completes necessary charting and record keeping in accordance with agency

and funding source requirements.

11. Coordinates and or provides education specific to identified medical needs.

12. Provides nutritional advising and support.

13. Attends and participates in appropriate meetings, psychiatric appointments

and clinical staffing's.

14. Administers injections to members when necessary.

15. Serves on agency committees as appropriate.

16. Secures medications in compliance with State and Federal regulations.

17. Performs other related duties in accordance with agency growth and changes.

Additional Essential Duties and Responsibilities for Methadone Clinic

1. Dispenses methadone dosages to members following State, Federal

policies.

2. Secures methadone and other medications in compliance with State and Federal

regulations.
