RN Case Manager

This is a 13-week contract position.

Schedule

8:00 AM - 4:30 PM, rotating every other weekend.

Job Description

The RN Case Manager will work in a fast-paced, high-acuity environment within a Level II trauma hospital. The candidate will be involved in a triad model with RN/SW/CMA team and collaborate with offsite UR team conducting Interqual assessments for initial and discharge planning.

Responsibilities

• Conduct initial assessments and ongoing evaluations of patient care needs.
• Coordinate patient care plans and discharge planning assessments.
• Collaborate with interdisciplinary teams to facilitate optimal patient outcomes.
• Utilize Meditech Midas for documentation and reporting.
• Ensure compliance with established regulations and evidence-based practices.

Required Experience / Certifications / Licensure

• RN license in Florida or Compact state.
• 2-4 years of experience as an RN Case Manager in an acute care facility.
• Experience with Meditech Midas and other clinical systems preferred.
• Adaptability to float between various units as needed.
