Job Requirements

• Active Registered Nurse (RN) license
• Associate degree required
• 3+ years of Home Health nursing experience
• 3+ years of OASIS experience
• Strong assessment, documentation, and care coordination skills
• Ability to work independently and manage a patient visit schedule
• Professional certification required
• Reliable transportation for home visits

Responsibilities

• Provide skilled nursing care to patients in the home health setting
• Complete comprehensive patient assessments and ongoing evaluations
• Develop and follow individualized care plans in collaboration with the care team
• Accurately complete OASIS documentation and other required clinical charting
• Educate patients and families on care plans, safety, and symptom management
• Coordinate care with interdisciplinary team members to support positive outcomes
• Travel between patient visits within the assigned coverage area
• Maintain a high standard of professionalism and compassionate patient care
