RESPONSIBILITIES:

Perform home visits for patients in a home care setting as delegated by the care-managing RN

Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in condition

Provides compassionate, patient-centered personal care including bathing/showering, washing hair, nails, continence care, etc. May also perform light housekeeping duties just as fixing a light meal, changing linen/making bed, empty trash, etc.

QUALIFICATIONS:

Current certification as a CNA in Connecticut REQUIRED

Valid driver’s license

Reliable transportation and willing to travel in New London County

Strong communication and interpersonal skills

Home health experience preferred
