Behavioral Health Care Manager

Humana is seeking a Field Care Manager, Behavioral Health to support members with complex mental health needs in the community. In this role, you will complete psychosocial assessments, develop person-centered care plans, and coordinate services across medical, behavioral, and social supports. You will collaborate with providers, families, and community resources to reduce hospitalizations, close care gaps, and promote recovery. The role includes field visits, telephonic and virtual outreach, documentation in care management systems, and participation in interdisciplinary team meetings to improve outcomes and member experience.
