Care Manager Registered Nurse

HCA Healthcare is seeking a Care Manager Registered Nurse to coordinate patient care across the continuum, ensuring safe, efficient, and patient-centered services. In this role, you will conduct assessments, develop and implement care plans, facilitate transitions of care, and collaborate closely with physicians, interdisciplinary teams, patients, and families.
