You will provide personalized therapy in the place where progress matters most, the patient’s home. Meaningful one‑on‑one patient care Work autonomy and flexible scheduling Supportive and collaborative clinical team Comprehensive onboarding and mentorship Tuition reimbursement for eligible full‑time employees Continuing education opportunities at no cost Opportunities for professional growth and advancement Medical, dental, and vision insurance 401(k) with employer match Paid time off and paid holidays Family and pet bereavement What You’ll Do Evaluate patients’ mobility, strength, balance, functional abilities, and home safety needs Develop and implement individualized Plans of Care Provide hands‑on therapy to improve mobility, independence, and quality of life Establish measurable treatment goals and monitor patient progress Adjust treatment plans based on changes in condition and functional outcomes Educate patients, caregivers, and family members on exercises, equipment, and safety Coordinate care with physicians, nurses, therapists, and other care team members Direct and support therapy assistants when applicable Complete accurate and timely evaluations, treatment notes, and clinical documentation Serve as a clinical resource and advocate for patients receiving therapy at home What You’ll Bring Graduate of an American Physical Therapy Association‑approved physical therapy program Current and unrestricted Connecticut Physical Therapist license At least one year of experience in a healthcare setting Strong assessment, communication, organization, and documentation skills Ability to work independently in patients’ homes Willingness to travel throughout Stamford and the assigned coverage area Must be able and willing to travel 50% Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations #J-18808-Ljbffr
