Physical Therapist (PT) – Home Health Per diems for the home health coverage areas of Souderton, Lansdale, Telford, Perkasie, and Quakertown. Evaluate the home environment. Assess for muscle strength, mobility, gait, ROM—potential for rehab. Direct physical therapy treatment. Instruct patients and families/caregivers in the use and care of therapeutic appliances. Determine priority needs for physical therapy. Report to physicians on patients’ reaction to treatment or changes in condition. May train patients in the use of prosthetic devices. Identify patient and family/caregiver needs for other home health services and referrals as necessary. Prepare and submit clinical and progress summaries based on the attainment of goals. Participate in discharge planning for patients. Prepare and submit a clinical progress summary based on the attainment of goals directed by Organization policy. Provide physical therapy consultation to home families/caregivers when indicated. Provide service education programs for nursing organization personnel as needed. Participate in the peer consultation process. Ensure that arrangements for equipment and other necessary items and services are available. Instruct, supervise and evaluate physical therapy aide care provided every fourteen days. Required Qualifications Current Physical Therapist license in the state of Pennsylvania. Graduate of an accredited Physical Therapy program. Strong clinical assessment, communication, and documentation skills. Valid driver’s license and reliable transportation. Current CPR/BLS certification. Minimum 1 year of clinical experience; home health experience preferred. Skills and Competencies Strong clinical assessment and treatment planning skills.
