Licensed Practical Nurse (LPN)

Job Description

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Long-Term Care/SNF team. The ideal candidate will provide high-quality patient care and support to residents, ensuring their well-being and comfort.

Key Responsibilities

Provide direct patient care and assist with daily living activities.
Administer medications and treatments as prescribed by healthcare providers.
Monitor patient health and report changes in condition to the nursing staff.
Maintain accurate and timely medical records using EMR systems.
Collaborate with other healthcare team members to ensure comprehensive patient care.
Assist in maintaining a safe and clean environment for patients and staff.

Shift Details

Shifts: Both 8-hour and 12-hour shifts available.
Primary hours: Variable, typically from 3:00 PM to 11:00 PM, with one 12-hour night shift from 7:00 PM to 7:00 AM each week.
Weekend coverage: Every other weekend required.
Holiday coverage: Every other holiday required.

Requirements

Minimum of 1 year of LPN experience in a Long-Term Care or Skilled Nursing Facility setting.
Current BLS Certification.
Strong communication and interpersonal skills.
Ability to work collaboratively in a team-oriented environment.
Flexibility to adapt to changing needs and schedules.

Additional Information

Assignment Length: 12 weeks.
Guaranteed hours: 40 hours per week.
Overtime may be available as per operational needs.
