Summary We are seeking a compassionate and knowledgeable Registered Nurse to join our Health Services team and support adults with developmental disabilities living in our residential programs. In this role, you will conduct participant health assessments, develop individualized medication self-administration programs, train and certify direct service personnel, and monitor medication practices to promote both safety and independence. Reporting to the Director of Health Services, you will work closely with participants, families, physicians, residential staff, and other members of the interdisciplinary team to coordinate care and maintain compliance with Illinois Department of Human Services requirements. This is a hands-on nursing role that combines clinical assessment, staff education, quality assurance, and participant advocacy. The Role Conduct annual health assessments for participants, documenting physical and mental health status, medical history, medication needs, and other relevant health information Review medication orders and prescribed medications to identify potential concerns, interactions, or areas requiring follow-up Monitor, document, and evaluate adverse drug reactions and coordinate with prescribing physicians when medication adjustments or additional interventions may be needed Participate as a member of community support teams to develop individualized medication self-administration training programs that promote participant independence and safety Develop and monitor medication self-administration programs in accordance with Illinois Department of Human Services standards for community-based developmental disability services Conduct medication administration training classes for direct service personnel to ensure competency in supporting participant medication programs Provide hands-on training, evaluation, and certification for staff who assist participants with medication self-administration Evaluate staff medication administration competencies annually or more frequently as needed, documenting outcomes and coordinating results with the Training Department Review medication error reports and implement appropriate corrective actions, including staff retraining, participant program modifications, or procedural changes Support quality assurance activities related to medication practices, physician communication, medication storage, and safekeeping procedures Maintain accurate and timely participant health records, medication documentation, training records, and competency evaluations in accordance with regulatory and agency standards Communicate and coordinate effectively with physicians, participants, families, direct service personnel, and interdisciplinary team members regarding participant health needs Respond promptly to medication concerns, adverse reactions, and other health-related issues while maintaining confidentiality and person-centered care Attend staff meetings, required trainings, and professional development activities to maintain clinical competency and agency knowledge Maintain required nursing and Developmental Disabilities Nurse Trainer credentials Travel locally between Lambs Farm residential sites, approximately 10% of the role, to complete assessments, training, and other nursing responsibilities Follow infection control protocols, medication handling procedures, confidentiality requirements, and all applicable health and safety standards Qualifications Bachelor of Science in Nursing from an accredited institution, Diploma in Nursing from an accredited three-year hospital school of nursing, or Associate Degree in Nursing from an accredited institution Current Illinois Registered Nurse (RN) license in good standing Minimum of one year of clinical nursing experience Experience working with adults with developmental disabilities preferred Current Developmental Disabilities Nurse Trainer certification through the Illinois Department of Human Services, or eligibility and willingness to obtain certification through paid on-the-job training Knowledge of medication administration protocols and safety standards applicable to community-based disability services Experience training others and evaluating staff competency in healthcare procedures Strong documentation skills with the ability to maintain accurate participant health records, training records, and competency evaluations Effective verbal and written communication skills with the ability to coordinate with participants, families, physicians, direct service personnel, and other members of the care team Ability to work collaboratively while maintaining professional and regulatory standards Basic digital literacy, including the ability to use email, electronic health records, web-based HR and timekeeping systems, and standard organizational communication platforms Ability to work primarily in residential environments with occasional office-based administrative responsibilities Flexibility to accommodate participant and program needs, including occasional evening or weekend availability Ability to stand during training sessions, walk between residential locations, and occasionally move training materials and medical supplies weighing up to 20 pounds Ability to safely operate standard medical equipment, including blood pressure monitors and thermometers, as required for participant health assessments Ability to work in environments involving participant health needs and potential exposure to illness while following appropriate infection control protocols Compensation The annual salary for this role is $64,000 - 72,000 per year Disclaimer: As part of the pre-employment process, all employees at Lambs Farm are required to successfully complete background and registry checks and medical screenings.
