Nurse Practitioner - Clinical Care Coordinator

The Clinical Care Coordinator - NP will be responsible for working alongside an experienced orthopedic surgeon within a dynamic and innovative academic pediatric orthopedic department in New York City.

This rapidly growing department offers collaborative practice opportunities to establish and expand ambulatory care services and participate in the development of new clinical programs within Pediatric Orthopedic Surgery.

Responsibilities

• Evaluates and treats pediatric and adolescent patients with a broad range of orthopedic and musculoskeletal conditions in the outpatient setting and other clinical settings as appropriate.

• Works independently and collaboratively with attending orthopedic surgeons, fellows, residents, nurses, therapists, social workers, and other multidisciplinary team members involved in the care of pediatric orthopedic patients.

• Provides independent patient care within the NP scope of practice, including new patient evaluations, follow-up visits, postoperative visits, and other appropriate clinical encounters.

• Supports increased patient access by providing additional clinical appointment availability and managing appropriate new, follow-up, and postoperative patient visits.

• Coordinates care for pediatric patients requiring non-operative treatment, including physical therapy, rehabilitation, bracing, imaging, pain management, and other appropriate services.

• Assists in managing and coordinating preoperative care to ensure patients are appropriately evaluated and optimized for surgery.

• Obtains appropriate consultations from other medical and surgical specialties and follows up on recommendations and clinical findings.

• Conducts peer-to-peer consultations with insurance carriers, as appropriate, to obtain authorization for diagnostic testing, procedures, medications, and surgical care.

• Rounds independently and/or collaboratively with attending physicians on pediatric orthopedic inpatients, as assigned.

• Prepares patient charts according to established clinical protocols and orders appropriate imaging and diagnostic studies through the Electronic Medical Record (EPIC).

• Evaluates and diagnoses pediatric orthopedic patients and develops appropriate treatment plans in collaboration with the supervising or attending physician when indicated.

• Orders, reviews, and interprets diagnostic and therapeutic tests based on the patient's condition and age-specific clinical needs.

• Prescribes appropriate pharmacologic and non-pharmacologic treatments within the provider's scope of practice and monitors the effectiveness of prescribed interventions.

• Performs appropriate office-based procedures within the provider's scope of practice and training.

• Removes and applies dressings, sutures, staples, slings, casts, braces, and other orthopedic devices as clinically appropriate.

• Evaluates postoperative patients for wound checks, recovery, and routine postoperative care and communicates abnormal findings or concerns to the attending surgeon.

• Reviews postoperative and discharge instructions with patients and families, including wound care, activity restrictions, pain management, medications, rehabilitation, and follow-up care.

• Responds to patient and family telephone calls and electronic messages at designated times throughout the day and documents all clinical actions in EPIC.

• Triages incoming clinical calls and messages, provides appropriate treatment instructions and education to patients and families, escalates concerns when necessary, and documents interventions in EPIC.

• Supports appointment management and clinical triage to ensure urgent and emergent pediatric orthopedic conditions are scheduled and evaluated appropriately.

• Processes medication and prescription requests in accordance with clinical protocols and documents actions in EPIC.

• Manages and oversees assigned attending physician EPIC In-Basket tasks to support timely communication and continuity of care.

• Completes necessary patient forms and clinical documentation, including school notes, disability and leave forms, activity restriction documentation, medical necessity letters, insurance inquiries, and other patient correspondence as appropriate.

• Assists pediatric patients and their families in meeting their healthcare needs by identifying potential or existing problems, discussing available treatment options, and providing education and support to facilitate informed healthcare decisions.

• Facilitates transitions of care within and between healthcare settings, including admissions, transfers, discharges, rehabilitation services, home care, and visiting nurse referrals when appropriate.

• Collaborates with physicians and clinical leadership in the development and implementation of patient care protocols, standardized workflows, and clinical pathways.

• Participates in quality improvement initiatives designed to improve patient access, clinical outcomes, patient experience, operational efficiency, and continuity of care within the Pediatric Orthopedic Surgery division.

• Assists with the development, coordination, and implementation of clinical research initiatives and clinical trials as appropriate.

• Participates in departmental and organizational initiatives, meetings, educational activities, and other responsibilities in accordance with established policies.

Minimum Qualifications

Education

• Master of Science in Nursing (MSN) required.

Experience

• Minimum of 3 years of nursing or related clinical experience.

Licensure & Certifications

• Active New York State RN and Nurse Practitioner licenses

• ANCC Board Certification

• Current BLS Certification

• DEA Certification
