Cordova Wellness & Rehab is seeking a compassionate, dependable, and organized Transportation Aide (CNA) to assist residents with medical appointments and transportation needs. This position is ideal for a Certified Nursing Assistant who enjoys working directly with residents while providing safe and reliable transportation services. Transportation Aide (CNA) Position Summary As the Transportation Aide (CNA) at Cordova Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include: Schedule resident medical appointments and coordinate transportation arrangements. Maintain an organized appointment calendar and communicate schedules with nursing staff and families as appropriate. Safely transport residents to and from medical appointments using the facility's company vehicle. Accompany residents to appointments and provide assistance, support, and advocacy as needed. Assist residents with entering and exiting the vehicle safely. Ensure all transportation documentation is completed accurately and timely. Communicate appointment outcomes and follow-up instructions to nursing staff. Perform CNA duties as needed while supporting resident care. Maintain a clean, safe, and well-maintained company vehicle. Maintain confidentiality of protected health information, including verbal, written and electronic communications.
