The full-time work hours are Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours required based on client needs.

Key Responsibilities\n

• Conduct comprehensive assessments of clients’ psychosocial needs, considering emotional, mental, and social factors.

• Develop and implement personalized care plans in collaboration with seniors and their families that reflect their specific goals and preferences.

• Provide counseling and emotional support to clients and their families, assisting them in navigating life transitions and challenges related to aging.

• Advocate for clients’ rights and help them gain access to essential community resources, ensuring they receive the necessary support and services.

• Monitor clients' well-being through regular check-ins and site visits, assessing living conditions and addressing concerns or safety issues.

• Facilitate communication among family members involved in care to resolve conflicts and provide clear information about the client's needs.

• Educate clients and their families about available supports, such as community resources and support programs aimed at enhancing quality of life.

• Maintain accurate and up-to-date documentation of all client interactions, assessments, and progress notes in the electronic health record (EHR) system.

Compensation and Benefits\n

The annual salary for this position ranges from USD 60,000 to 100,000, commensurate with experience and education.
