Stroke Program Coordinator Works closely with the multidisciplinary stroke team to develop, implement, and evaluate standards of care and protocols to improve patient outcomes and practices within Billings Clinic. Provides leadership for nursing care and management of adult patients diagnosed with stroke, ensures evidence‑based care, develops continuing education, and coordinates performance improvement and research. Essential Job Functions Advance bedside nursing practice to improve patient and family needs assessment and educate on acute stroke and stroke prevention. Collaborate within multidisciplinary team to develop, implement, and evaluate standards of care and protocols to improve patient care, practice, and systems. Collaborate with clinical leaders to ensure a well‑coordinated, multidisciplinary program of care and education across settings. Integrate related research into practice, educate organization on clinical trials, and track research while participating in stroke research. Identify key outcomes specific to stroke self‑care management, analyze current educational initiatives, plan for improvements, and systematically integrate them into practice within Billings Clinic system, coordinating with regional sites for continued improvement on stroke education and evidence‑based best practice standards. Support and model behaviors consistent with Billings Clinic’s mission, vision, values, code of business conduct and service expectations; meet all mandatory organizational and departmental requirements; maintain competency in all applicable standards. Maintain knowledge and adhere to standards and criteria designated by DNV and American Heart Association (AHA)/American Stroke Association (ASA) "Get With The Guidelines" to maintain stroke accreditation. Collect, review, and analyze information for evaluating patient care outcomes; maintain stroke registry for performance improvement. Facilitate registry design to support performance improvement activities, trend reports, and research while protecting confidentiality. Coordinate stroke care management utilizing effective communication across continuum of care, including planning and implementation of clinical protocols/practice management guidelines, monitoring care of in‑hospital patients, serving as a resource for clinical practice, and discharge planning. Identify multidisciplinary opportunities for improvement in a timely manner and assist team members with problem resolution through plan development, implementation, and evaluation. Monitor quality, continuity of care, clinical outcomes and system issues related to care delivery; develop quality filters, audits, case reviews, identify trends and sentinel events, and outline remedial actions while maintaining confidentiality. Coordinate stroke committee and multidisciplinary stroke peer review meetings. Participate in case review, standardize practice guidelines, and direct community stroke education and prevention programs. Participate in development of stroke care systems at community, state, and national levels. Stabilize complex network of many disciplines that work together to provide high‑quality care; serve as internal resource for physicians and staff across Billings Clinic departments; act as liaison for stroke care with community and state. Work with Stroke Medical Directors to develop and evaluate stroke evidence‑based guidelines, pathways, and protocols to reduce inappropriate variation in care. Participate in research selection, analysis, and dissemination of findings; facilitate protocol design for accurate data collection, feedback, and analysis. Identify needs and set goals for own growth and development; meet all mandatory organizational and departmental requirements. Facilitate and ensure comprehensive multidisciplinary collaboration of stroke care occurs with patient, family, physicians, and other health care providers; identify fragmentation/duplication and assist team in effective resource utilization. Exhibit exceptional knowledge in stroke patient clinical care; maintain competency in all organizational, departmental, environmental, and safety standards relevant to job performance. Provide expert nursing care in expanded role to patients with stroke. Consult with nursing, pharmacy, medical staff, and others to develop, implement, and evaluate comprehensive plan of care, including discharge plan, to achieve desired patient outcomes. Verbal communicate pertinent information with key clinicians in timely manner; utilize appropriate documentation tools for effective written communication. Provide intermittent reinforcement of stroke education to patients and family. Consult with all team members across continuum to ensure optimal patient transition plans. Participate intermittently in Acute Stroke Code activations to ensure protocols are followed. Provide post‑hospital contact to evaluate transition and outcome. Provide telemedicine program support with new site establishment and existing site support; ensure highest standards in patient/family education. Provide and support stroke education for Billings Clinic and regional staff development. Role model best practice in stroke education; inspire inpatient clinical staff to engage actively in stroke prevention education. Develop and operationalize a philosophy of learning for patients and families with stroke. Utilize empowerment model to create patient partnerships; employ strategies that foster self‑management for stroke prevention. Personalize educational interventions to match patient and family learning needs, barriers, and preferences. Consult with clinical staff concerning nursing care in patients who have experienced stroke. Establish collaborative relationships with physician staff and other team for effective interdisciplinary management within hospital and beyond. In conjunction with Stroke Program Manager, develop and deliver educational programs for all staff who care for patients who have experienced stroke. Work across disciplines and physical areas to ensure best practice consistency for patients who have experienced stroke. Assist in providing interdisciplinary awareness of American Stroke Association and DNV guidelines. Design and implement processes that contribute to efficient, cost‑effective care for patients who have experienced stroke. Design and make available pertinent learning tools and resources. Assist in outreach efforts to inform greater community regarding stroke. Minimum Qualifications Bachelor's degree in nursing. Five years of professional nursing (one within Billings Clinic) in a related cardiac care area such as critical care, ambulatory telemetry unit, or emergency services. Current RN license in the state of Montana, at hire. National Institutes of Health Stroke Scale (NIHSS) certification required or to be obtained within first year of employment. Healthcare Provider and ACLS CPR certifications, at hire.
