Senior Care Coordinator

3 days ago Be among the first 25 applicants Direct message the job poster from Quality Placement Authority Conduct initial assessments and regular re-assessments of clients’ physical, mental, and social needs. Develop, implement, and monitor individualized care plans in collaboration with clients, families, and providers. Coordinate services including medical care, home health, transportation, meal delivery, and community programs. Serve as the primary point of contact for clients and families to navigate the healthcare and social service systems. Advocate for clients’ rights and ensure they receive culturally competent and respectful care. Collaborate with physicians, nurses, case managers, and other care team members. Document all client interactions, services provided, and care updates in compliance with organizational policies and regulatory standards. Identify gaps in care or service delivery and initiate corrective action. Support crisis intervention and discharge planning when necessary. Participate in interdisciplinary care team meetings. Train and mentor junior care coordinators or case managers as assigned. Bachelors or Masters degree in a human service field.
