Seven Healthcare are seeking an experienced Physical Therapist for a contract assignment in Winston Salem, NC. This role requires expertise in home health physical therapy, offering the opportunity to impact patient care directly in a supportive and dynamic environment.

- Review patient records for diagnosis, medical history, precautions, and contraindications
- Establish measurable goals and develop treatment plans for patient independence/function
- Re-assess treatment effectiveness and modify plans as needed
- Communicate patient needs and progress to treatment team and involved parties
- Consult with medical providers about evaluation, progress, and discharge plans
- Recommend adaptive equipment to enhance the rehab program
- Maintain and submit documentation according to regulations and standards
- Supervise Physical Therapist Assistants and Aides in patient care
- Participate in staff development and obtain continuing education
- Active NC PT license
- Experience in Home Health PT
- Proficiency in Oasis and HCHB
