In your role, you will be responsible for evaluating patients, whether in an inpatient or outpatient setting, and designing tailored treatment plans to address their specific needs. You'll implement physical therapy interventions that enhance mobility, strength, balance, and overall function. Collaboration will be key as you work alongside nurses, physicians, case managers, and a variety of rehabilitation professionals to ensure the best outcomes for your patients. You will take the time to educate patients and their families about therapeutic goals and vital exercises for home. Proper documentation of evaluations and progress will be critical, adhering to both facility and regulatory requirements. Active participation in care team rounds and interdisciplinary conferences will also assure coordinated care.

To be considered for this role, candidates must possess a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program. State licensure is required, and while CPR certification is preferred, any specialized certificationssuch as in the fields of neurology or geriatricswould certainly be a valuable asset, though they are not mandatory. We seek a Physical Therapist who brings previous experience and is able to treat patients with complex medical and mobility challenges. Strong communication and collaboration skills are essential, along with comfort in using digital charting and clinical documentation systems.
