RN Case Manager - Telephonic / Field - South Central and South West Kansas

About This Role The RN Case Manager – Telephonic / Field role involves managing care for members with complex and chronic needs in South Central and South West Kansas. This position is ideal for experienced nurses who thrive in a field-based environment and are committed to optimizing member health care. Highlights Location: Overland Park, KS Schedule: Full-time, Monday through Friday, 8am – 5pm What You'll Do Perform care management within the scope of licensure for members with complex and chronic care needs. Conduct assessments virtually and in person to identify individual needs. Develop and implement care management plans to address objectives and goals. Facilitate authorizations/referrals as appropriate within benefits structure. Coordinate internal and external resources to meet identified needs. Monitor and evaluate the effectiveness of care management plans. Interface with Medical Directors and Physician Advisors on treatment plans. Negotiate rates of reimbursement as applicable. Assist in problem solving with providers, claims, or service issues. Travel 10-15% within South Central to South West Kansas to visit members.
