We’re looking for a Care Coordinator who is proactive, empathetic, and thrives in a people-centered environment.About the RoleThe Care Coordinator is the heart of our client care operations - ensuring each client receives consistent, high-quality care tailored to their needs and lifestyle. You’ll work closely with families, caregivers, and community partners to create seamless, well-supported care plans. This role blends organization, heart, and leadership to keep our clients safe, supported, and comfortable in their homes.Key ResponsibilitiesServe as the primary point of contact for new client referrals and care transitionsCoordinate personalized in-home care plans with compassion and attention to detailEnsure continuity and consistency of care by matching clients with the right caregiversMonitor client satisfaction and well-being through regular check-ins and visitsSupport families with guidance, resources, and care planning insightSupervise and support caregivers, including performance evaluations and ongoing communicationCollaborate with healthcare professionals, insurers, and internal teams to streamline servicesParticipate in rotating on-call responsibilities to maintain service continuityWho You AreYou are a naturally warm, highly organized professional with a heart for helping others.
