Key Responsibilities Conduct comprehensive psychosocial assessments of clients to identify their emotional and social needs. Develop individualized care plans in collaboration with clients and their families, reflecting their unique goals and preferences. Provide counseling and emotional support to clients during critical life transitions, such as bereavement or health challenges. Advocate for client access to necessary community resources and services, ensuring they receive the support required for improved well-being. Engage with multidisciplinary teams, including healthcare providers and caregivers, to enhance client care and support. Monitor client progress through regular follow-ups, addressing any emerging issues or safety concerns related to living conditions. Facilitate family meetings and communication to resolve conflicts and enhance collaborative care. Educate clients about community resources, support groups, and programs that can foster social engagement. Maintain thorough and up-to-date documentation, including client assessments and care plans, in accordance with organizational policies and legal regulations. Stay current with best practices, ethical standards, and regulatory requirements in the field of social work. Patient Population / Client Focus In this role, you will focus primarily on older adults, many of whom may face significant life changes, chronic health conditions, or social isolation. You will be crucial in providing support that directly improves their emotional and social well-being. By understanding the unique challenges seniors encounter, you will advocate for their needs, ensuring they maintain a high quality of life and access to essential resources. Qualifications A Master's degree in Social Work from an accredited institution. Current licensure as a Licensed Master Social Worker (LMSW) in the state of Florida. A minimum of two years of relevant experience in social work, especially within community or geriatric settings. Strong understanding of social work principles, aging processes, and available community services. Exceptional interpersonal skills and the ability to build rapport with clients and families. Effective communication skills that facilitate collaboration with interdisciplinary teams. Competence in using electronic health record (EHR) systems and familiarity with various healthcare technologies. Preferred Experience Experience working with seniors or in community-based services. Familiarity with mental health and crisis intervention strategies. Proven ability to navigate community resources and support networks effectively. Compensation and Benefits We offer a competitive annual salary ranging from $60,000 to $100,000, commensurate with experience and education.
