A well-established skilled nursing facility is seeking a compassionate Social Service Director in the Streamwood Area.
This leadership role is ideal for someone who thrives on making a difference in the lives of seniors and their families, while also guiding a team of dedicated staff.
Key Responsibilities:
- Oversee and coordinate social services for both short-term rehab and long-term care residents
- Conduct psychosocial assessments and care planning
- Serve as a liaison between residents, families, and interdisciplinary teams
- Ensure compliance with federal, state, and facility policies
- Support residents' emotional, mental, and social well-being through advocacy and guidance
Qualifications:
- Bachelor’s degree in Social Work or related field (LSW preferred)
- Previous experience in a skilled nursing or healthcare setting
- Strong knowledge of state and federal regulations (MDS, discharge planning, etc.)
- Excellent communication and organizational skills
- Leadership or supervisory experience preferred
