Care Coordinator/Case ManagementProvide targeted mental health case management for youth with serious emotional disturbances and co-occurring disordersMeet with enrolled and potential clients and their families to create a Plan of CareConduct comprehensive assessments and other assessments as required by DHMHCoordinate and facilitate Family Team MeetingsProvide management of the POCsCollect information during the application process and as identified in each POCIdentify providers, supports, and resources for clientsCoordinate meetings with client and family to (a) meet with providers to ensure goodness of fit for proposed services and products in compliance with the POC (b) meet with family peer-to- peer support, intensive in-home service, mobile crisis response service, and other providers appropriate to the POCMaintain client clinical documentation using EHRProvide on-call services as scheduledPromptly respond to client crisesProvide case management for client open-access as scheduledFacilitate groupsMinimum Qualifications:Bachelor's degree in a mental health field and 1 year of mental health experience, including mental health peer support; or Bachelor's degree in a field other than mental health and 2 years of mental health experience, including mental health peer supportAbility to handle stressful situationsMust maintain a flexible work schedule to meet scheduling needs of clientsMust meet Care Coordinator training and certification requirementsMust complete the training required by CSA and BHA, including CASII, ESCII, and CANSMust complete Wraparound certificationJob Type: Full-timePay: $45,000.00 - $52,000Benefits:401(k)Dental insuranceEmployee assistance programEmployee discountHealth insuranceLife insurancePaid time offVision insuranceSchedule:8-hour shiftMonday to FridayHybridLanguage: Bilingual a plusLicense/Certification:Driver's License (Required)Work Location: In person
