The Pharmacist Pharmacy role reports to the Director of Pharmacy and supports community, long-term care, and compounding practice environments. This position provides safe and effective medication services, including prescription verification, medication dispensing, immunizations, medication therapy management, chronic disease consultations, compounding services, and collaborative patient care.

Responsibilities

• Verify prescriptions for accuracy, appropriateness, and completeness.
• Dispense medications and provide patient counseling.
• Screen for drug interactions and conduct drug utilization reviews.
• Provide medication therapy management and chronic disease consultations.
• Administer immunizations and support preventive care services.
• Perform non-sterile compounding and provide compounding services.
• Participate in multidisciplinary clinical rounds, collaborative practice agreements, and routine care-team collaboration.
• Maintain prescription documentation, inventory control, pharmacy operations, and regulatory compliance.
• Support medication management services in long-term care settings.

Required Qualifications

• Doctor of Pharmacy (Pharm.D.) degree.
• At least 3 years of experience in pharmacy practice.

Preferred Qualifications

• Active pharmacist license.
• Active pharmacist licenses in New Jersey and New York.
• Immunization certification.
• Experience with medication therapy management and prescription verification.
• Experience with drug utilization review and pharmacy information systems.
• Experience in inventory management and patient counseling.
• Strong communication skills.
