Physical Therapist - Travel Contract

Physical Therapist — Home Health — Pomona, CA Looking for a change of scenery and a chance to help others. Provide skilled physical therapy services in a home health setting to patients recovering from injury or managing chronic conditions. This opportunity is located in Pomona, CA and supports clinicians seeking a community-based practice environment. Minimum Qualifications Doctoral, Master’s, or Bachelor’s degree in Physical Therapy from an accredited program approved by the APTA. Active California physical therapy license required prior to start of assignment. At least one year of home health physical therapy experience required. BLS certification from American Heart Association or American Red Cross may be required. Valid driver’s license and reliable transportation for patient visits in the community. Experience with HomeCare HomeBase (HCHB) is preferred but not required. Preferred Qualifications Prior travel or per diem home health experience preferred. Experience working with post-acute care or complex medical populations preferred. Assignment Details Shift hours: 8:00 AM to 5:00 PM. Schedule: Monday through Friday, 8-hour shifts. No on-call or weekend requirements. Caseload includes patients in Pomona and surrounding communities such as Yorba Linda, Orange, Anaheim Hills, and Santa Ana.
