Position Overview

We are seeking an experienced and highly skilled Contract Speech Therapist II to join our dynamic home health team. This position requires a seasoned professional who can independently manage a full caseload from the first day of assignment, delivering exceptional speech-language pathology services to geriatric patients in their homes. The ideal candidate will possess advanced clinical skills, particularly in bedside swallow evaluations, and demonstrate the ability to work autonomously while maintaining the highest standards of patient care.

As a Contract Speech Therapist II, you will play a vital role in improving the quality of life for our elderly patients by assessing, diagnosing, and treating speech, language, cognitive-communication, and swallowing disorders. This position offers the flexibility of contract work while providing the rewarding experience of making a meaningful difference in patients' lives within the comfort of their own homes.

Key ResponsibilitiesClinical Assessment and Evaluation

• Conduct comprehensive speech-language pathology evaluations in patients' homes, including detailed assessments of speech, language, voice, fluency, cognitive-communication, and swallowing functions
• Perform bedside swallow evaluations to assess dysphagia and aspiration risk, making critical recommendations for diet modifications and safe swallowing strategies
• Utilize standardized assessment tools and clinical observation to establish baseline function and identify areas requiring therapeutic intervention
• Document evaluation findings thoroughly and develop individualized treatment plans based on assessment results and patient goals

Treatment and Intervention

• Provide evidence-based speech therapy interventions tailored to each patient's specific needs, abilities, and home environment
• Implement therapeutic exercises and strategies to address dysphagia, including compensatory techniques, diet texture modifications, and swallowing exercises
• Deliver cognitive-linguistic therapy for patients with dementia, stroke, or other neurological conditions affecting communication and cognition
• Train patients and caregivers in therapeutic techniques, compensatory strategies, and home exercise programs to maximize functional outcomes
• Modify treatment approaches based on patient progress, tolerance, and changing medical conditions

Documentation and Compliance

• Complete all clinical documentation in accordance with Medicare guidelines, state regulations, and agency policies within required timeframes
• Prepare detailed evaluation reports, progress notes, discharge summaries, and recertification documentation
• Maintain accurate records
