Position Details:Hartford HealthCare is seeking a full time Family Nurse Practitioner (APRN) or Physician Assistant (PA) to join our hybrid remote primary care team within HHC Medical Group.In this role, you will provide Transition of Care, Annual Wellness Visits, or same-day primary care visits remotely. Depending on the schedule and patient needs, a nurse or medical assistant will support you either on site or remotely by managing in person tasks when required.You will also have scheduled time to be on site, where you will support the clinical team, but see patients virtually at two of our Campus Care locations to deliver traditional primary care services.This model allows you to work with a skilled and flexible team of Advanced Practitioners while helping expand Hartford HealthCare's growing virtual care services.RoleOpen to Family Nurse Practitioners (FNP) and Physician Assistants (PA) LocationSetting: Hybrid – virtual/remote from home plus two days in person at two of our Campus Care sites, TBD based on candidate's residence.
