Clinical Nurse CoordinatorThe Clinical Nurse Coordinator provides clinical leadership and daily coordination of patient care activities within the Department of Medicine. The role serves as a clinical resource and liaison among patients, providers, nursing staff, clinical support staff, and department leadership.The Clinical Nurse Coordinator provides direct nursing care, clinical assessment and triage, patient education, care coordination, and clinical follow-up; supports safe and efficient clinical operations; facilitates staff education and competency; and assists the Nurse Director with quality, patient safety, regulatory compliance, and clinical workflow improvement.The Clinical Nurse Coordinator provides clinical guidance and operational support to clinical staff and escalates clinical, operational, and staff concerns to department leadership as appropriate.Duties & Responsibilities1. Provides clinical nursing care, assessment, triage, and patient education.Performs and documents nursing assessment and clinical care within RN scope of practice and established department workflows.Provides clinical triage of patient symptoms and concerns, determines appropriate urgency, and escalates to the provider or department leadership when indicated.Provides patient and family education regarding plan of care, test/procedure preparation, medications, treatment instructions, self-care, and available resources.Administers medications and performs nursing interventions in accordance with orders, policy, patient-identification requirements, and applicable nursing standards.Supports timely clinical follow-up and communicates changes in patient condition or care needs to the appropriate provider.2. Coordinates patient care and serves as a clinical resource.Serves as a clinical point of contact for patients and families and coordinates care across providers, departments, services, and care settings.Coordinates referrals, testing, procedures, medication-related needs, and other clinical services appropriate to the assigned specialty area.Supports continuity of care through timely communication, follow-up, and resolution of clinical concerns.Collaborates with providers, nurses, medical assistants, schedulers, and other disciplines to facilitate safe and efficient patient care.3. Supports daily clinical operations, delegation, and workflow management.Provides day-to-day clinical leadership and assists in coordinating clinical support staff and patient-care activities.Serves as a point person for clinical workflow questions, operational barriers, and patient-care concerns.Appropriately delegates or assigns tasks based on staff role, scope of practice, demonstrated competency, patient needs, applicable policies, clinical standards, and established organizational requirements.Assists with clinical staffing coordination and assignment adjustments as directed by the Nurse Director and according to departmental needs.Identifies clinical workflow or operational concerns and collaborates with department leadership and appropriate stakeholders to implement solutions.4. Supports staff education, onboarding, competency, and performance development.Participates in onboarding, orientation, training, and ongoing education of nursing and clinical support staff.Provides real-time clinical guidance and coaching and reinforces clinical standards, policies, workflows, and safe-practice expectations.Assists with the development, implementation, documentation, and monitoring of required clinical competencies, certifications, and credentials.Serves as a clinical resource and identifies clinical practice or performance concerns for escalation to the Nurse Director or appropriate leadership.May provide relevant observations and input regarding staff performance; formal corrective action and employment decisions remain the responsibility of management.5. Supports quality, patient safety, and regulatory compliance.Promotes compliance with patient-safety requirements, including patient identification, effective communication, medication safety, infection prevention, confidentiality, and other applicable standards.Assists in identifying, reviewing, and escalating patient-safety events, near misses, clinical risks, and process concerns in accordance with organizational policy.Participates in audits, monitoring, corrective action plans, quality-improvement activities, and regulatory readiness initiatives.Monitors adherence to clinical policies and established standards and communicates identified gaps to department leadership.6. Coordinates clinical workflows and process improvement.Assists in developing, reviewing, implementing, and evaluating clinical workflows, procedures, algorithms, and practice standards.Coordinates implementation of new or revised clinical procedures and workflows and supports associated staff education.Evaluates clinical processes for safety, efficiency, consistency, and patient access and recommends improvements to the Nurse Director.Collaborates with providers, department leadership, clinical informatics, and other stakeholders to address electronic health record, clinical messaging, documentation, and workflow needs.Supports clinical workflows involving messages, orders, refills, results, documentation, and other specialty-specific processes as assigned.7. Supports specialty-specific clinical programs and services.Maintains clinical knowledge and competency appropriate to assigned specialty area(s) within the Department of Medicine.Coordinates specialty-specific patient care, education, testing, procedures, and follow-up as applicable.Serves as a resource to patients, providers, and staff regarding specialty-specific clinical workflows and care needs.Supports required clinical documentation, consents, and specialty-specific patient-care processes.Supports development and improvement of clinical services and programs in collaboration with the Nurse Director, providers, and operational leadership.8. Provides clinical leadership and administrative support to the Nurse Director.Assists the Nurse Director in assessing clinical needs and developing education, workflows, action plans, and improvement initiatives.Provides timely communication regarding significant patient-care, staffing, workflow, safety, or regulatory concerns.Assists with collection, monitoring, and reporting of clinical, quality, operational, and patient-care data as assigned.Participates in interdisciplinary meetings, department initiatives, recruitment/interview activities, and other clinical leadership functions as requested.9.
