- Conduct thorough evaluations of patients in both inpatient and outpatient settings and compose personalized treatment plans
- Implement physical therapy techniques aimed at enhancing mobility, strength, balance, and overall functional capacity
- Work collaboratively with nurses, physicians, case managers, and fellow rehabilitation professionals to enhance patient outcomes
- Coach patients and their families about therapeutic objectives, discharge procedures, and exercises for home
- Maintain detailed documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory standards
- Engage in care team discussions and interdisciplinary meetings to guarantee coordinated care efforts
- Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution
- Licensure as a Physical Therapist within the state
- CPR certification preferred
- Specialized credentials (e.g., neurological, geriatric) considered a plus but not mandatory
- Prior experience is highly preferred
- Capability to address patients with intricate medical and mobility challenges
- Strong communication and collaboration skills
- Familiarity with digital charting and clinical record-keeping systems
