Physical Therapist (PT) / Physical Therapist Assistant (PTA)

We are seeking dedicated and compassionate Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to join our growing team.

Job Summary

As a member of our home health team, you will provide skilled physical therapy services to patients in their homes. You will work closely with physicians, nurses, caregivers, and other healthcare professionals to develop and implement individualized treatment plans that promote recovery, mobility, and independence.

Responsibilities

Physical Therapist (PT)

• Evaluate patients and develop individualized plans of care.
• Perform comprehensive physical therapy assessments.
• Provide skilled therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Monitor patient progress and modify treatment plans as appropriate.
• Educate patients and caregivers on home exercise programs and fall prevention.
• Complete accurate and timely clinical documentation.
• Coordinate care with physicians and the interdisciplinary healthcare team.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state and federal regulations.

Physical Therapist Assistant (PTA)

• Deliver physical therapy treatments under the supervision of a licensed Physical Therapist.
• Assist patients with therapeutic exercises, gait training, balance activities, and functional mobility.
• Observe and report patient progress to the supervising PT.
• Reinforce home exercise programs and patient education.
• Maintain accurate and timely documentation.
• Follow all company policies and home health regulations.

Qualifications

Physical Therapist (PT)

• Graduate of an accredited Physical Therapy program.
• Current state Physical Therapist license in good standing.
• Current CPR/BLS certification preferred.
• Valid driver's license, reliable transportation, and current auto insurance.
• Home health experience is preferred but not required.
• Strong communication, organizational, and time management skills.

Physical Therapist Assistant (PTA)

• Graduate of an accredited Physical Therapist Assistant program.
• Current state PTA license in good standing.
• Current CPR/BLS certification preferred.
• Valid driver's license, reliable transportation, and current auto insurance.
• Home health experience is preferred but not required.
• Excellent communication and patient care skills.
