Key Responsibilities\n

In your role as an LCSW, you will be responsible for:

  • Conducting comprehensive assessments to understand each client's mental health challenges and needs.
  • \n
  • Developing personalized treatment plans that incorporate best practices in evidence-based interventions such as cognitive-behavioral therapy (CBT) and solution-focused therapy.
  • \n
  • Implementing therapeutic interventions and continually monitoring and adjusting treatment plans to optimize patient outcomes.
  • \n
  • Maintaining accurate and confidential clinical records, adhering strictly to HIPAA regulations and organizational policies.
  • \n
  • Collaborating with a multidisciplinary treatment team to ensure holistic care, connecting clients to additional resources such as medical support, dietary consultations, or community services as necessary.

  • Providing education and emotional support to patients and their families regarding mental health conditions and available treatments.
  • \n
  • Engaging in professional development to stay updated on the latest mental health research and treatment methodologies.

Patient Population / Client Focus\n

Your practice will predominantly focus on individuals and families facing complex emotional and psychological challenges due to chronic illness, grief, or end-of-life transitions. In this role, you will have the opportunity to impact the lives of clients in a hospice setting, providing them with the support they need to navigate their emotional journeys. You will foster a safe and nurturing environment where clients feel heard and understood while working to help them achieve their treatment goals.

Qualifications\n

The ideal candidate will possess the following qualifications:

  • A Master’s degree in Social Work from an accredited institution.
  • \n
  • A valid LCSW license in the state of Illinois.
  • \n
  • A minimum of three years of clinical experience, particularly in hospice, palliative care, or an outpatient mental health setting.
  • \n
  • Demonstrated proficiency in solution-based psychotherapy interventions and familiarity with various therapeutic techniques.
  • \n
  • Excellent communication skills, both verbal and written, with the ability to build rapport with clients and work effectively within a team.
  • \n
  • A collaborative spirit, capability to work well in a team-oriented environment, and strong problem-solving skills.

Preferred Experience\n

While not required, preference will be given to candidates with:

  • Prior experience working in a hospice or end-of-life care setting.

Compensation and Benefits\n
