RN Case Manager PRN

Swedish Medical Center, a pioneer institution under HealthONE, is looking for a highly skilled and compassionate RN Case Manager PRN to join our dynamic team. In this role, you will be integral in managing patient care with a focus on enhancing the quality of patient management, maximizing satisfaction, and promoting cost effectiveness. The successful candidate will be responsible for the comprehensive assessment of patient and family needs, planning of targeted transition and discharge, implementation of care plan, and evaluation of outcomes. Working in collaboration with physicians, nurses, and other healthcare professionals, you will ensure the continuity of patient care and optimal patient outcomes.
