Job Description

We are seeking a dedicated and experienced Home Health Physical Therapist to provide personalized care to patients in their homes in East Portland and surrounding areas. The ideal candidate will demonstrate a strong commitment to providing high-quality therapy while managing patient needs efficiently.

Responsibilities

• Evaluate and develop personalized treatment plans for patients.
• Conduct home visits, adhering to productivity standards.
• Provide skilled physical therapy treatments and interventions.
• Document patient progress and adjust treatments as necessary utilizing EPIC and Oasis systems.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience.
• Valid Physical Therapist license as per state regulations.
• Basic Life Support Certification (BCLS) required.
• Oasis experience is required.
