Responsibilities include: Completing admissions, social service evaluations, providing referrals, and maintaining communication with residents and families. Assisting with financial and legal matters such as pensions, legal referrals, and preplanning arrangements. Providing discharge planning services, including coordinating placements and working with outside agencies. Arranging counseling services and psychiatric medication management as needed. Ensuring accurate and timely documentation in MDS care plans.
