Physical Therapist - Travel Contract

Join a traveling Physical Therapist role in Shelbyville, KY, providing home health services to patients recovering from injuries and managing chronic conditions. This position requires completing SOCs/ROCs, routine visits, supervisory visits, and re-assessments while documenting in OASIS and HCHB.

Minimum Qualifications

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Kentucky Physical Therapist license required to start the assignment.
• Home health experience required.
• Experience with OASIS and HCHB required.
• BLS certification from AHA or ARC may be required.
• OASIS certification preferred.
• Must be able to complete SOCs/ROCs, routine visits, supervisory visits, and re-assessments.

Preferred Qualifications

• OASIS certification.

Assignment Details

• Shift hours: Monday–Friday, 8:00 AM–4:30 PM.
• Weekly guaranteed hours: 32.
• On-call or weekend requirement: No.
• Will be reimbursed for mileage between visits from first patient to last patient.
• Coverage areas include Shelby County, eastern Jefferson County, and Spencer County.
