About This Role The Care Manager develops, assesses, and facilitates complex care management activities primarily for members with mental and behavioral health needs, aiming to provide high-quality, cost-effective healthcare outcomes. Highlights Location: Remote, GA Residency: Must reside in Georgia Schedule: Full Time, Day shift What You'll Do Evaluate member needs via phone or in-home visits and recommend/facilitate care plans Perform telephonic, digital, home, and/or other site visits outreach to assess member needs Develop ongoing care plans for members with high acuity and identify necessary providers and resources Coordinate between members, families, community resources, and care provider teams Monitor care plans and member status for changes in treatment and provide recommendations Facilitate care coordination and collaborate with providers to ensure timely access to services Collect, document, and maintain member information to ensure compliance with regulations Provide education to members and families on healthcare options and benefits Provide feedback to leadership on opportunities to improve care delivery Perform other duties as assigned Requirements Education Master's degree in Behavioral Health or Social Work, or Degree from an Accredited School of Nursing Experience 2 – 4 years of related experience Licenses & Certifications One of the following licenses required:
- Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC), OR
- RN based on state contract requirements with BH experience
