RN - Case Manager

Providence Saint Joseph Medical Center - Burbank is seeking an RN - Case Manager. This travel position is scheduled from 08:00 AM to 04:00 PM, five days a week for a total of 40 hours per week. The contract duration is 13 weeks, starting on 08/10/2026 and ending on 11/09/2026. The role includes a weekend requirement, typically one full weekend per four weeks, subject to department needs.

The candidate must have two years of experience in acute medical care management, performing care coordination and discharge planning. Certifications and skills in acute hospital, admission criteria, care coordination, CMS, CPT coding and billing, discharge planning, disease management, HIPAA guidelines, ICU, MS, needs assessment/order DME, plan of care are required. The candidate must also commit to working three of the five holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, and New Year’s Day.

Two references covering one year within the last three years are required. The local radius for references is 50 miles. All travelers are expected to float between all facilities within a given state. If required for the assignment, the client agrees to supply communication devices at no cost to the provider or agency. Providers will be responsible for submitting monthly cell phone bills directly to the agency.
