Location: Boynton Beach, FL
Job Type: Full-Time | Direct Hire
Pay Rate: Open to Discussion
Setting: Home Health

Key Responsibilities

Oversee and coordinate clinical staff and patient care services.
Supervise patient assessments, reassessments, and individualized care plans.
Coordinate care with physicians, patients, caregivers, and clinical teams.
Supervise, train, mentor, and evaluate clinical employees.

Requirements

Graduate of an accredited School of Nursing.
Active Registered Nurse (RN) license in Florida.
Current CPR certification.
Minimum 2 years of RN experience.
At least 1 year of management experience in home care, hospice, or a comparable healthcare setting.
Required hands-on Medicare Home Health experience , including OASIS assessments, documentation, and compliance.
Valid driver's license, auto insurance, and reliable transportation.
