RN - Case Management

Acute Care Case ManagerThis is a 13-week contract position.Day shift, 5x8-hour schedule from 08:00 - 16:30. Variable days off with efforts made for consecutive days off.The Acute Care Case Manager will focus on care coordination for patients in an acute care setting.
