Job Description

Home Health Physical Therapist (PT) – Seaside, OR & Surrounding Areas

Assignment Details

• Shift: 8-Hour Day Shifts
  • Facility cannot accommodate 4x10s or 3x12s schedules
• Patient Ratio: 1:1 patient care
  • May visit up to 6 patients per day
  • Standard productivity expectation: 30 productivity points/day

Experience Requirements

• Minimum 1 year of Skilled Home Health experience required
• First-time travelers accepted

Certifications & Licensing

• BCLS Certification required
• Pending license accepted
• Oasis experience required

Schedule & Call Requirements

• Weekend Requirement: None
• On-Call Requirement: None
• Floating Between Units: Not required
• Block Scheduling: Not available

Traveler Requirements

• Must have reliable personal transportation
• Daily travel radius may extend up to approximately 60 miles
  • Estimated total drive time: 60–90 minutes/day
• Facility groups patient visits to minimize travel time

Vaccination Requirement

• Facility requires COVID-19 primary vaccination series plus one booster
