Nurse Practitioner Job Summary: Carease Health - IL is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a Nurse Practitioner at Carease Health - IL, you will play a critical role in our comprehensive preventative care program, leveraging Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to prioritize proactive measures for enhanced physical health and well-being.
