Home care can be demanding, but it offers a uniquely rewarding experience for nurses who value autonomy and patient-centered care./ppService Areas: Tampa, Brandon, Valrico, Lakeland, Mulberry, Dover, Lithia, Riverview, Ruskin, Sun City Center, Seffner, and more./ph2Key Responsibilities/h2ulliProvide skilled nursing care during home visits within assigned territories/liliAdminister medications, treatments, wound care, and other ordered procedures/liliDeliver individualized care while evaluating patient response to treatment/liliFollow physician orders and the established plan of care under RN supervision/liliMonitor patient conditions and promptly report changes to the clinical team/liliComplete accurate, timely documentation in accordance with agency standards/liliPerform patient assessments as required/liliAssist with activities of daily living when appropriate/liliEducate patients and families on care needs, treatment plans, and disease management/li/ulh2Qualifications Skills/h2ulliActive, unrestricted Florida LPN license/liliGraduate of an accredited nursing program/liliMinimum 1 year of nursing experience preferred/liliPrior home health experience strongly preferred/liliExperience with: Pediatrics, adults, and geriatrics Feeding tubes (G/J tubes) Developmental and neurological disorders Endocrine conditions and injections Tracheostomy care/liliStrong clinical judgment and ability to work independently/liliEffective time management and organizational skills/liliProficiency with EMR systems (experience with Kantime preferred, but not required)/liliSolid documentation and communication skills/liliUnderstanding of infection control, safety standards, and patient confidentiality/li/ulh2Additional Requirements/h2ulliValid drivers license and reliable transportation with active insurance/liliCurrent BLS/CPR certification/liliRecent statement of health including TB screening/liliAbility to travel to assigned cases/li/ulh2Why Join Our Team/h2pWe are a nurse-led and operated agency, built with a strong understanding of both patient care and the realities of clinical practice.
