Social Services Director

A skilled nursing facility in the Cincinnati area is seeking an experienced Social Services Director to join its leadership team. This position plays an important role in supporting residents and families throughout their stay while coordinating psychosocial needs, care planning, discharge planning, and resident advocacy. Responsibilities Oversee the facility’s Social Services department and resident psychosocial services Complete required assessments, documentation, and care planning Coordinate discharge planning and connect residents and families with appropriate community resources Participate in interdisciplinary care plan meetings Support residents and families through changes in condition, placement needs, and other concerns Maintain accurate and timely documentation in accordance with facility and regulatory requirements Collaborate closely with Nursing, Admissions, Therapy, and other departments Assist with survey preparation and maintain compliance with applicable long-term care regulations Qualifications Previous long-term care/skilled nursing experience required Previous Social Services experience in a healthcare setting required LSW preferred, but not required PointClickCare (PCC) experience preferred Strong knowledge of resident rights, discharge planning, care planning, and psychosocial needs Excellent communication, organization, and documentation skills Ability to work effectively with residents, families, and an interdisciplinary team What We Offer Comprehensive benefits package Paid time off and Paid Holidays Supportive leadership team Opportunity to make a meaningful impact on residents and families #J-18808-Ljbffr
