Speech-Language Pathologist (SLP) – Home Health Flexible Schedule | Full-Time | Part-Time | PRN Immediate Opening Home Health Speech Therapist (SLP) Responsibilities Assess, evaluate, and provide professional services prescribed by the physician to homebound patients. Assist physician in evaluating the patient to determine type of speech or language disorder and appropriate corrective therapy. Develop rehabilitation plan based on evaluation and goals, in consultation with physician. Provide rehabilitation services for speech and language disorders. Qualifications & Competencies Completion of a speech-language pathology curriculum in accordance with state Speech-Language and Hearing Practice Act. Actively licensed to practice as a speech-language pathologist in the state. Current CPR certification. Valid auto insurance and state driver license. Consistently demonstrates professionalism through appearance, performance, and communication. Demonstrates safe competencies to provide care to patients of all ages, respecting patient rights. Ability to regularly complete evaluations, technical treatments, and restorative procedures common in visit-based speech therapy practice. Observe and record patient reaction to treatment and changes in condition; notify physician and appropriate health professionals of any changes. Maintain confidentiality of patient assessments and medical records. Communicate effectively with patient/family and other members of the health care team and agency staff. Attend case conferences and make referrals to other disciplines as indicated. Consistently adhere to standard precautions, transmission precautions, aseptic techniques, and infection control. Instruct healthcare team members and families in methods of assisting patient to improve communication. Communicate with RN/Therapist regularly. Meet with Director of Patient Services regularly.
