Physical Therapist Assistant - Home Health

Schedule: Shift: 8-hour days

Assignment Details:

• Caseload: Up to 6 patients per day
• Productivity expectation: ~30 points per day
• Guaranteed hours: Facility may call off up to one shift per pay period

Requirements:

• 1 year of skilled home health experience
• Must have reliable personal transportation
• BCLS certification required
• Clinical skills in home health PT including gait training, use of assistive devices, post-op orthopedic care, neuro rehab, and prosthetic management

Roles & Responsibilities:

• Provide physical therapy services to patients in their homes
• Evaluate patient needs and develop treatment plans
• Implement interventions to improve mobility, strength, and overall physical function
• Document patient progress and modify treatment plans as necessary

Compliance / Notes:

• Dress code: Business casual (no jeans); scrubs allowed (no specific color)
• Free parking available
• Confirm time-off restrictions with hiring manager
