In-Home Traveler Physician (Full-Time) - Connecticut State and Surrounding Markets

Signify Health, part of CVS Health®, is seeking a full‑time physician to provide in‑home health evaluations across Connecticut and other licensed states. The role involves traveling 100% of the time to visit patients in their homes, conducting comprehensive assessments, and connecting patients with appropriate care. Responsibilities Conduct in‑home visits lasting about 45 minutes, including medication and medical history review, physical evaluation, and any ordered diagnostic tests such as diabetic eye exams or spirometry. Use an iPad and the intuitive clinical workflow for each evaluation. Travel locally and within licensed states, up to 100% of the time, including overnight stays, car rentals, and flights as required. Receive per‑completion incentive pay for all in‑home visits and additional reimbursement for ordered diagnostic and preventive services. Participate in virtual visits as needed. Qualifications MD or DO with an active, unrestricted license in the coverage area. Ability to obtain and maintain multi‑state licenses in the required coverage areas. Coverage areas within Connecticut include Greater Bridgeport, Naugatuck Valley, Western Ct, South Central Ct, Capitol Region, Lower Ct River Valley, Northwest Hills Ct, Northeastern Ct and Southeast Ct. Willingness to travel extensively, including overnight stays and interstate travel. Comfortable performing duties in diverse home environments, including those with pets, secondhand smoke, or allergens. Compensation Base pay for each in‑home health evaluation is $145.00. Compensation may vary based on experience, education, geography, and other factors. Additional reimbursement is available for completed diagnostic and preventive services as ordered. Work Environment As an in‑home clinician, the primary workplace is the private residence of patients. The role requires adaptability to varied home settings while maintaining patient safety and quality of care.
