Health/Human Service Case Manager II

Join DSAAPD as a Health/Human Service Case Manager II where you will be responsible for case management at the full performance level. You will provide counseling and assistance for clients to meet their personal, social, health, and economic needs. Through applying a person-centered planning approach, you'll make home and field visits to interview clients, their families, and/or professionals to create a plan of care that supports the least restrictive setting. Your duties will also involve making referrals; assisting in the application process for a variety of programs; and determining initial and/or ongoing eligibility for services under the Community Services Program.
