Community Health Worker (CHW)

Location: San Jacinto, CA (Travel between San Jacinto & San Bernardino)

Type: Full-Time | Regular

Pay: $25/hr

Are you passionate about making a real impact in your community? St. John's Community Health (SJCH) is looking for a Community Health Worker (CHW) to connect with local residents, spread health knowledge, and help people access the care they need.

As a CHW, you'll be the bridge between healthcare and the community—meeting people where they are, sharing resources, and building relationships that truly matter.

What You'll Do:

• Community Outreach: Host events, knock on doors, call residents, and collaborate with local organizations to connect people to health programs.

• Health Education: Lead workshops, create educational materials, and provide guidance on topics like nutrition, chronic disease management, and preventive care.

• Patient Advocacy & Care Coordination: Help community members navigate healthcare, connect them with resources, and coordinate with providers.

• Data & Reporting: Track outreach activities, analyze trends, and report on community health needs.

• Event Planning & Stakeholder Engagement: Plan health fairs and campaigns while building strong connections with community leaders and organizations.

What We're Looking For:

• High school diploma or equivalent (Bachelor's in Public Health or related field preferred)

• Experience in community outreach or health education

• Strong communication & interpersonal skills

• Knowledge of community resources & healthcare services

• Cultural competence & ability to work with diverse populations

• Bilingual (English/Spanish) a plus, but not required

• Microsoft Office skills & ability to work independently and in a team
