The Benjamin Healthcare Center, a premier residential, long-term, rehabilitation, and subacute care provider, is seeking a dedicated and compassionate LPN Licensed Practical Nurse to join our team in Boston, MA.
About the Role:
As a valued member of our healthcare team, the LPN Licensed Practical Nurse will play a vital role in delivering high-quality patient care, working closely with our interdisciplinary team to develop and implement individualized care plans for our residents. Your exceptional patient-centered approach, attention to detail, and commitment to delivering outstanding care will make a meaningful difference in the lives of those we serve.
Responsibilities:
Provide direct patient care, including administering medications, monitoring vital signs, and performing routine treatments
Conduct routine assessments and evaluations to identify changes in patient condition and document accurately
Collaborate with the healthcare team to develop and implement individualized care plans
Educate patients and families on treatment options and disease management
Maintain accurate and detailed records of patient care and treatment
Participate in quality improvement initiatives to promote positive patient outcomes
Demonstrate a strong commitment to patient-centered care, confidentiality, and patient rights
Requirements:
Current LPN license in the state of Massachusetts
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Commitment to delivering exceptional patient care and customer service
