Job Summary The Service Linkage Worker provides services within a broad range of focus areas to assist patients in accessing and adhering to care. Services provided may include HIV testing and counseling, patient assessments, assistance to reduce access and adherence barriers, follow‑up to ensure referrals are completed, patient navigation assistance, and coordination with case managers and other clinic staff. Minimum Qualifications Bachelor’s degree from an accredited college or university with a major in social or behavior sciences. Documented paid work experience in providing client services to Persons Living with HIV/AIDS (PLWHA). 1 year of documented paid experience may substitute 1 year of college. A minimum of 1 year paid work experience with PLWHA. Equipment Computer and general office equipment. Special Requirements Communication Skills: Above Average Verbal; Writing/Composing: Correspondence, Reports. Other Skills: Analytical, P.C., MS Word. Work Schedule Flexible, Eligible for Telecommute. Other Requirements Completion of Texas Department of Health, HIV Case Management 101 within first 90 days of employment.
